DOD has replaced the use of the SF 93 with the DD Form 2807-1 and 2807-2. DD Form 2807-2 is used by recruiters to "pre-screen" applicants. Any significant medical problems found by use of this form requires recruiters to get permission from MEPS before recruits can process. The DD Form 2807-1 is the medical form used by MEPS during the MEPS medical examination. The DD Form 2808 is used by the MEPS Medical Staff to document the medical examination.
